The team won the Integrated health care category in the Western NSW Local Health District's Living Quality and Safety Health and Innovation awards announced last week.
The project, titled Blayney partnerships in action, aimed to support the provision of health care for the Blayney community.
It identified gaps in services and worked with the Blayney Family Medical Practice, Orange Aboriginal Medical Service, Blayney Council and Bathurst Health Service to address the situation to improve service for the community.
The focus was on enhancing the quality of care for Blayney people.
About 70 projects entered the 11 award categories.
Topics included outreach maternity care, distraction therapy for patients with cognitive impairment and new information technology project.
It was part of May being Innovation month across Western NSW health services.
Chief Executive Scott McLachlan said the awards recognised and rewarded quality work by staff to improve the quality of health care throughout the region.
"It allows us to openly share ideas about ways we can improve the health and wellbeing of people in our region," he said.
"The number of projects that will be shared with all staff across the month is a reflection on the initiative, expertise and dedication our staff have in the work they do.
The awards were presented at a gala dinner in Dubbo on May 18.
