The strategy is centred around providing patients with chronic diseases, or the possibility of developing them, with an integrated care plan.
The Blayney Integrated Care team is holding a free community information forum on Thursday, December 8 to discuss the growing chronic disease burden in our community and how the strategy can help tackle the issue.
Blayney is one of nine towns in Western NSW selected by NSW Health to implement a new integrated, holistic approach to care to help prevent and address chronic diseases such as diabetes, lung and heart disease.
Everyone in Blayney is invited to the interactive forum on the Thursday 8 December from 6.30-8.30pm in the Blayney Community Centre Church Street.
The Forum will have guest speakers discussing what chronic disease is and how to minimise risks.
The forum will also discuss how the integrated care team can help residents manage their health.
“This forum is a great opportunity for people to chat with a range of health professionals and learn what they could be doing better to stay healthy and out of hospital,” said Kathleen Hillier, Health Service Manager for Blayney MPS.
“People can also sign up to our free Integrated Care program that can help them better understand their condition, help manage appointments and medication and help them access a range of services – social and health related – so they can stay well.”
Local GPs Dr Vijay and Dr Samaan are strong supporters of Integrated Care and will be at the forum to answer any questions the community might have.
Community nurses, exercise physiologists, pharmacists and representatives from the Orange Aboriginal Medical Service, Western NSW Primary Health Network, Centrepoint Sport and Leisure will also be at the forum.
